Families and Friends:

We want to inform you of the following  confirmed cases of COVID-19; 1 employee and 18 residents. Three of the 18 residents are in the hospital. 

We also have 6 residents on Patient Under Investigation (PUI) precautions. 

The safety of our residents and staff is our top priority. If your loved one are diagnosed with COVID-19, we will notify you directly via telephone. To reduce the spread and impact of COVID-19, we continue to follow guidance from the Centers for Disease Control and Prevention (CDC) and the Center for Medicare & Medicaid Services (CMS).

We are staying in close communication with local and state health officials to ensure we are taking the appropriate steps under the current circumstances, including:

·        Enhanced infection control precautions, such as becoming vaccinated, increasing the availability of hand sanitizer, and more frequent cleaning and disinfecting of high-touch surfaces. 

·        Screening residents and staff for COVID-19 symptoms, as well as anyone else who must enter our facility.

·        Testing staff and residents for the virus causing COVID-19 for those that are showing signs and symptoms. 

·        Providing and requiring facemasks for staff and visitors. 

·        Providing facemasks for residents, if tolerated and following CDC guidance.

Bibb County’s positivity rate continues to remain below 10%. We are at 4.73% for this week. We
continue to not have any documented cases of Covid in our building at this time. We certainly do not
want to give it to your family/residents. Your help is greatly appreciated. Please check in at front lobby

test positive. As always we monitor staff and residents. We are no longer testing residents 2 x a week.
We however continued to test all staff weekly. The % rate for the county is below 10%. We certainly do

This week’s positivity rate is 18.1%.  We had 1 employee test positive for Covid this week, and 4 residents remain  positive and 0 residents in isolation for exposure.  Residents with covid have not had any serious symptoms at this time.  As always we monitor staff and residents. We continue testing residents 2 times a week for at least 2 weeks. . And we continued to test all staff 2 x a week as long as we have staff positive /residents positive and the % rate for the county is above 10%.. This week’s positivity rate is 23.8%.  We had 1 employee test positive for Covid this week, and 4 residents test positive and 3 residents in isolation for exposure.  Residents with covid have not had any serious symptoms at this time.  As always we monitor staff and residents. This week’s positivity rate is 24.5%.  We had 2 employees test positive for Covid this week, and 1 resident test positive and 3 residents in isolation for exposure.   We continue testing residents 2 times a week for at least 2 weeks. . And we continued to test all staff 2 x a week as long as we have staff positive /residents positive and the % rate for the county is above 10%.. This week’s positivity rate is 26.5%.  We had another employee test positive for Covid this week, and 0 residents test positive  or any  residents are in observation for exposure to covid.. quarantine due to exposure at this time.   We continue testing residents 2 times a week for at least 2 weeks. Resident that was in isolation is no longer in isolation . . And we continued to test all staff 2 x a week as long as we have staff positive /residents positive and the % rate for the county is above 10%.. This week’s positivity rate is 27.8%.  We had another employee test positive for Covid this week, and 0 resident. No residents are in quarantine due to exposure at this time.   We continue testing residents 2 times a week for at least 2 weeks. Resident that was in isolation is no longer in isolation . . And we continued to test all staff 2 x a week as long as we have staff positive /residents positive and the % rate for the county is above 10%.. No residents are in quarantine due to exposure at this time.   We continue testing residents 2 times a week for at least 2 weeks. This past week and next week. And we continue to test all staff 2 x a week as long as we have staff positive /residents positive and the % rate for the county is above 10%.. This week’s positivity rate is 30.3%.  We had an employee test positive for Covid this week and 2 residents.  The residents do not have symptoms but have been placed in quarantine.  The employee does have symptoms and has been placed on quarantine also. The State is keeping up with COVID positivity rate on a weekly basis now.  This week’s positivity rate is 27.8%.  We have had 1 employee test positive for Covid. We continue to test Staff 2 times a week, because the % rate is over 10%.  We are now testing residents 2 times a week for at least 2 weeks.  All residents have tested negative except for one. We continue to test Employees and residents 2x a week.  Please be mindful of any symptoms you may have.  . This week’s positivity rate has gone up to 18.6%.  The % positivity rate continues to climb.  Staff is being tested 2 x’s a week.  Apparently, COVID is on the rise again.  We have had 1 employee test positive for Covid on 6/07/22.  She has since tested negative and returned to work. This week’s positivity rate has gone up to 14.9%.  The % positivity rate is climbing again.  Staff is being tested 2 x’s a week.  Apparently, COVID is on the rise again.  We have had 1 employee test positive for Covid, therefore we are testing residents 2 times a week for 2 weeks.  If no one else tests positive by  this Friday June 10, 2022 we will no longer test residents twice a week. The Covid % rate in BIBB County is still very high. As pf today it is 33.8%. We are testing staff 2 times a
week and residents 2 times a week. As of now we have 9 residents in our COVID unit and 2 staff

January 18, 2022
As of today, the COVID % RATE IN Bibb County is 43.5%. We have had several employees test positive
for COVID. They have been sent home until tested -. All Staff have returned except 2. We now have 4
residents that have tested Positive for COVID. We have set up an area for COVID + Residents. No
visitors are allowed in this area and COVID + Residents are on Isolation. You can still do phone calls and
window visits. However, the windows must REMAIN CLOSED at all times. We are sorry for any
inconvenience this may cause. We are trying our best to keep our residents safe.


January 13, 2022
Covid % rate in Bibb County was 45.6%. This is very high. Almost half the population of Bibb County has
COVID. CMS said we had to open up the facility to visitors. Please be mindful and wear a mask, use
hand sanitizer and do social distancing while in the facility. Make sure you screen as you come in the
building.

December 30,2021
As of yesterday the % of Covid in Bibb County was 21.5%.the resident Council of our facility at Fountain
Blue Rehab and Nursing voted to shut the facility down. MEANING NO VISITATION allowed at this time.
Only Essential personnel will be allowed. You can still make window visits, BUT windows cannot be
opened. The Resident Council feels this is the safest thing to do at this time. We are very sorry for any
inconvenience this has brought during the Holidays. However, Our resident’s safety comes first. We
will keep you informed as information changes. Have a safe and good Holiday.

As the 2021 year comes to a close, Covid % for Bibb County is on the rise again. Today the % rate has
gone up to 7.2%. Please follow the CDC guidelines for helping to prevent Covid. There is another
Variant on the rise. For the safety of our residents, your loved ones, PLEASE wear face coverings and
keep a social distance of 6 feet. Wash your hands frequently and/or use hand sanitizer. Have a safe and

at 15.5%. It still remains above 10%. We will continue to test staff 2 x a week. No new positive cases

However you can still visit at the windows, but the windows must stay closed. We apologize for any

As of September 22, 2021, Bibb County’s % rate has dropped below 20%. It is 16.9%. This is still above
10%; therefore we will continue to test staff 2 times a week. We have not had any staff or residents test
positive since August 31, 2021. . We are still not allowing visitors in the building. However you can still
visit at the windows, but the windows must stay closed. We apologize for any inconvenience this may
cause. Please remember you can visit your loved one at their window. Due to the guidelines of CDC we
cannot open windows, not even to pass items to and from residents. Please call the facility during
regular business hours and someone will meet you at the front door. This is for everyone’s safety.


September 14, 2021
As of September 14, 2021 Bibb County % rate is still above 20% at 22%.Up to date we have had 10
employees test positive since July 13, 2021. All of these employees have returned to work after the
quarantine period and have tested negative. We have completed the testing of residents twice a week.
We continue to test staff twice a week due to the fact Bibb County’s % COVID rate is above 10%.
Therefore we no longer have any Residents or staff testing positive in the building at this time. We are
still not allowing visitors in the building. However you can still visit at the windows, but the windows
must stay closed. We apologize for any inconvenience this may cause.


August 31, 2021
As of August 31, 2021 Bibb County’s Covid % rate is 23.1%. Up to date we have had 10 employees test
positive since July 13, 2021. 6 of these employees tested positive on Monday and Tuesday of this week
and 5 of these 6 were fully vaccinated with Pfizer vaccine in Jan and Feb of this year. Our resident that
we had that was in-house that was positive for covid passed away on Saturday. She was admitted as
positive and was on hospice services at time of admission. We are testing the staff and residents 2x
weekly and no residents have tested positive on our testing on August 31. We currently do not offer
visitation inside the lobby but family my visit at the closed windows.


AUGUST 25, 2021
As of August 25, 2021 Bibbs County’s Covid % rate is 21.8%. Up to date we have had 4 employees test
positive since July 13, 2021. We have 1 active case of covid in the building. Resident was tested for
COVID upon admission and tested positive. Therefore admitted with it. We have placed Resident in
Quarantine and keeping the resident away from all other residents, We are no longer allowing visitors
in the building.

You can still make window visits, However the windows cannot be open. We are sorry for any
inconvenience this may cause. OUR RESIDENTS are our first priority.

COVID 19 UPDATES
As of August 18, 2021, Bibb County’s rate is less than 20% at 20.9%. We have had 2 employees test
positive for COVID – 19 at this time. We have begun testing residents 2 times a week for the next 2
weeks. For this week and next week. Staff will be tested 2 times a week until % drops below 10%. At
this time NO RESIDENTS have tested positive. We plan to continue to limit visitation to the front lobby.
Visitors have to call and make an appointment for during the day. Visitors are limited to 2 at the time.
And the visitors have to stay at least 6 feet from other families that are visiting. We apologize for any
inconvenience this may cause. However, Are Residents’ are our main concern.
As of August 3, 2021 Bibb County’s COVID % rate is over 20%. We will continue to test staff 2 times a
week. Screen all visitors at front door. No visitors will be allowed passed the front door. All visitors are
asked to wear masks in the building, including the front lobby. We encourage the residents not to open
the windows. If a resident is on Hospice and emanate we can let 2 visitors in to see them 30 minutes at a
time. We apologize for any inconvenience this may cause. We are trying to protect our residents.
Bibb County’s COVID % rate continues to be over 10 %. As of today Bibb County is at 15.9%. Bib county
% rate has increased from 3.2% on 7/09/21 to 15.9% today. We have been testing Residents once a
week for 2 weeks. This ends today. And testing staff 2 times a week. As you know 2 weeks ago we had
a staff member test positive for COVID 19. No one else has tested positive. We will continue to test
staff 2 x a week until % rate falls below 10%. Unless we have another positive case we will no longer be
required to test residents on a regular basis. We continue to limit visitation on the halls. You can still
visit residents in the front lobby with an appointment. You must continue to wear masks, check in in
lobby, and each group has to stay 6 feet apart. We are sorry for any inconvenience this may cause
anyone, but our main priority is our residents. We must keep them safe. Thank you for your
cooperation in advance.


Bibb County is above 5%, we are considered in the yellow zone now by county % rate. At this time we
average around 7.9%. Since our rate is below 10%, we test our staff for Covid 19 monthly. Due to one
of our therapists testing positive for COVID last Thursday night, July 15, 2021 we are testing Staff and
residents at least twice a week for 2 weeks. Then we will go back to testing staff once a month. If the %
goes up to 10% we will go back to testing staff once a week. This could change on a daily basis. As of
today none of our residents have tested positive. We are still in forcing the wearing of masks and hand
hygiene in facility and no visitors beyond the front door.


We have not had an active case of Covid 19 in our facility since February 2021.


As of July 1, 2021: The new strain of COVID DELTA has been reported within 45 miles of our facility.
Staff will no longer reuse masks or wear cloth masks. When arriving for work they will check in as usual
and get them a new mask. This strain seems to be worse than the original. It is more contagious. We
are going by CDC guidelines to prevent this strain from entering our facility. We apologize for any

inconvenience this may cause to our Residents, Families, visitors and staff. However our first
responsibility is to our residents and staff.


Residents can still have visitation in the front lobby. Scheduled visits will decrease exposure.


Reopening for visitors was MARCH 1, 2021.

You must call in advance to set an appointment time for visitation
Visiting hours will be between 10:00 a.m. and 3:00 p.m. 7 days a week
Only 2 family members per resident will be allowed to visit at a time
Visitation will be limited to 30 minutes. And no visitation in rooms unless otherwise approved by
the Administrator. THIS IS ONLY FOR RESIDENTS THAT ARE AT END OF LIFE!
We will limit to only 6 families allowed in the facility at a time
All visitors must wear a mask. PLEASE no cloth masks. If you need a mask there are some at
the front of the building where you screen when you come in.
All visitors will be asked to sign and answer a few questions and have your temp logged. Temp
must be <100.0 to be allowed to visit.
You will enter and exit through the front entrance only.
Social distancing will be required during visits. We are asking to please refrain from hugging and
touching at this time. The Delta version of COVID is highly contagious and can travel further
than the recommended 6 feet with the COVID – 19.
You will visit with your loved one in the front lobby. No one is allowed in resident rooms at this
time unless okayed per Administrator.
Hand sanitizer will be available and we ask that you please utilize it.


Tables and chairs will be sanitized between visits for everyone’s protection
If at any time we develop a positive case of Covid-19 or DELTA for residents or staff, visitation
we will have to reassess how we are doing visitation.


Please understand we are doing our best to keep our residents, your loved ones, safe.


The facility continues to follow all CDC/Department of Health guidelines to mitigate the
exposure/spread of COVID-19.


The facility continues to frequently clean and disinfect common areas and high-touch surfaces.
Staff and all visitors continue to be screened daily for signs and symptoms and temperatures
recorded. Staff continue to wear masks. Residents are encouraged to encouraged/reminded to
wear when out of their room..


New admissions and readmissions will be quarantined for 14 days per CDC Guidelines if they
cannot prove they have been vaccinated. Group activities are beginning to return to normal.
Insuring residents are 6 feet apart. Dining room has been opened up for Fine Dining as long as
proof of vaccination and/or Residents are seated at least 6 feet apart.
